Dale M. Gallagher, D.D.S. Facial & Oral Surgery practice makes it our goal to provide you with the finest available surgical care. At the same time, we realize that decisions for your health are often related to financial concerns, especially when choosing an elective procedure that may not be covered by ordinary insurance plans. We want you to know that we do our best to provide the most convenient financial options, and that we will work hand in hand with you to maximize your insurance reimbursement for covered procedures. Remember, our team is here to help you with ALL your concerns, both medical and financial. We have made special arrangements with Capital One Healthcare Finance to have an easy application procedure for financing, should you choose to go forward with a plan of care that requires that extra help.

These general financial policies have been established for our business, which we believe will prevent misunderstandings about financial issues as well as allow payment flexibility that some patients need. Discussion of financial issues early in your treatment process will prevent most concerns, misunderstandings, and stress related to payment for the services you receive.

General information/Insurance

• As a courtesy to our patients, we will file claims to primary insurance on all visits and procedures, whether they are delivered in our office or the hospital.
• Please remember insurance coverage Is a contract between the patient and the insurance company. The ultimate responsibility for your fees to your doctor rests with you.

Referrals

• For TMJ and Orthognathic patients, you (patient/insured member) are required to:
  1. Know whether or not your MEDICAL INSURANCE requires a referral to a specialist (oral surgeon) non-contracted provider, and
  2. to obtain that referral before you are scheduled to see our physician.
• You should take the time to call your insurance company to ask specifically about the doctor you wish to see and your covered benefits.
• Referrals typically have an expiration date and limited number of visits so you should be careful to monitor the dates and visits.
• Our office is not able to obtain this referral for you, please be sure you have a valid referral prior to your first visit with our office.

We understand that individual situations may make it difficult to meet these financial expectations and wilt be happy to discuss our financial options with you.

There is a discount for surgical procedures when payment is made with cash or a check you will receive a 5% discount.

We also accept payment by credit and debit cards (MasterCard, Visa and Discover).

Our office offers financing arrangements, including non-interest bearing loans through Capital One Healthcare Finance, which in many cases can be combined with treatment fees with your restorative dentist and/or orthodontist as well. This allows you to have the convenience of one payment for your dental care/treatment for multiple offices.

Patients with TRICARE or MEDICARE must sign an OPT OUT form understanding that our office will NOT be filing claims to either of these insurance companies and with MEDICARE the patient is not allowed to file for the services rendered.
