First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options.

Please arrive 15 minutes prior to your appointment so that we may prepare your patient chart and coordinate relevant information for Dr. Gallagher.

Please also assist us by providing the following information at the time of your consultation:

• Your surgical referral slip and any x-rays from your dentist or physician. If your dentist or physician has taken x-rays, you may request that they forward them to our office prior to the appointment. They may be emailed to us if they are in a digital format to: info@jawpain.com
• A list of medications you are presently taking. Including herbal supplements and over-the-counter medications.
• Bring your current medical and dental insurance cards. This will allow us to process any claims.
• Driver's license or a state issued ID. If the patient is a minor, the patient's parent or guardian are responsible for providing this information.
